Project: National Autistic Society/Norfolk County Council – Transitions materials
STCOM were commissioned by Norfolk County Council to produce a suite of materials for children with disabilities to help explain the often complex transition process between Children’s and Adult Services. The materials, including separate guides for young people, their parents and schools, were developed with extensive consultation and use carefully selected language and original illustrations throughout. We were contacted by the National Autistic Society who asked us to repackage the Transitions Passport for Young People for their use nationally, which with Norfolk County Council’s blessing, we did. Six years on and updated versions of these documents are still in use.
